Description
Issuance of certified copies of birth, marriage, death, and other civil registry documents from the Municipal Civil Registrar. The MCR verifies records through CRIS/Registry Book and prepares the certificate. Fee: ₱115 per copy. For PSA endorsement: additional ₱200. Various MCR forms are available depending on document availability and status.
Requirements
- MCR Form 1A (Birth authorization) / 1B (Birth not available) / 1C (Birth destroyed)
- MCR Form 3A (Marriage) / 3B (Marriage not available) / 3C (Marriage destroyed)
- MCR Form 2A (Death not available) / 3C (Death destroyed)
Procedure
- Request certification from MCR. Staff verifies through CRIS/Registry Book and prepares certificate (15 min).
- MCR certifies the facts stated as to correctness.
- Proceed to Treasury Office for payment of ₱115 per copy.
- Present OR to MCR Office. Documents released.
- For PSA endorsement: MCR verifies and prepares endorsement (₱200, 10 min).
