Birth Certificate

Description

A birth certificate is a vital record that serves as proof of identity, age, and Filipino citizenship. The Municipal Civil Registrar (MCR) of Mapandan issues certified copies of locally registered birth certificates. This document is required for school enrollment, passport application, employment, and other legal purposes.

Requirements

• Valid government-issued ID of the requesting party
• PSA copy of the birth certificate (if available, for verification)
• Barangay certification of residency (for the person named on the certificate)
• Affidavit of two disinterested persons (required for delayed registration only)

Procedure

1. Visit the Municipal Civil Registrar’s Office and present valid identification.
2. Fill out the request form and provide the necessary supporting documents.
3. Pay the certification fee at the Municipal Treasurer’s Office.
4. Receive the certified birth certificate upon release.
