Description
A death certificate is an official document that records the death of an individual. It is required for settlement of estates, insurance claims, pension benefits, and other legal matters. The Municipal Civil Registrar (MCR) of Mapandan issues certified copies of locally registered death certificates.
Requirements
- Medical certificate or certificate of cause of death
- Valid government-issued ID of the requesting party
- Barangay certification (for the requesting party’s relationship to the deceased)
Procedure
- Visit the Municipal Civil Registrar’s Office and present valid identification.
- Fill out the request form and provide supporting documents.
- Pay the certification fee at the Municipal Treasurer’s Office.
- Receive the certified death certificate upon release.
