Certificate of Indigency

Get a Certificate of Indigency from the MSWDO for hospital discounts, legal aid, scholarships, and other social services.

Description

The Certificate of Indigency is issued by the MSWDO to indigent families certified by the Barangay Captain. This document is required to avail of various services including hospital discounts, legal assistance from the Public Attorney's Office (PAO), scholarship assistance for college education, and other social welfare programs. The MSWDO conducts an interview and assessment to verify the client's eligibility before issuing the certificate.

Requirements

  • Barangay Certificate of Indigency
  • Medical Certificate (if hospitalized)
  • Police report / Police blotter / RTC/MTC report (if applicable)
  • Report Card Form 138 / Recommendation (if for students)
  • Birth Certificate (PSA/MCR)

Procedure

  1. Register at the MSWD Office. Staff will refer you and conduct an initial interview.
  2. Submit your Barangay Certificate of Indigency and other supporting documents.
  3. Staff will conduct an interview and assess your eligibility for assistance.
  4. Receive your Certificate of Indigency upon approval.
