Description
A Social Case Study Report is issued to clients who need to avail of assistance from PCSO, other government hospitals, institutions, NGOs, DSWD, and various foundations. The report is prepared by MSWDO social workers after a thorough assessment that includes an initial interview, home visit, and evaluation of the client's situation. The report serves as a supporting document for various forms of assistance.
Requirements
- Certificate of Indigency from Barangay Captain
- Medical Certificate / Clinical Abstract / Medico-Legal report
- Police report / Police blotter (if applicable)
- Whole body picture
- Birth Certificate (PSA/MCR)
- Psychiatric Evaluation Report (as needed)
Procedure
- Apply at the MSWD Office. Staff will refer you for initial interview.
- Discuss your problem and provide relevant information. Staff conducts initial interview and gathers data.
- Staff may conduct a home visit or follow-up, especially for CICL cases.
- Submit Barangay Certificate of Indigency and other required documents.
- MSWDO prepares and computerizes the Social Case Study Report.
- Receive your copy of the Social Case Study Report upon approval.
