Description
The Population Office facilitates group enrollment of indigent residents to PhilHealth under the Local Government Unit (LGU)-sponsored program. The LGU subsidizes the premium contributions of qualified beneficiaries. Enrolled members receive health insurance coverage for hospitalization and medical services at PhilHealth-accredited facilities. The Population Office verifies eligibility, collects required documents, and submits the enrollment to PhilHealth.
Requirements
- Valid Government-issued ID
- Certificate of Indigency from the Barangay Captain
- Birth Certificate or any valid ID showing date of birth
Procedure
- Visit the Population Office and submit the required documents.
- Staff verifies eligibility and reviews the documents (3 min).
- Staff fills out the PhilHealth enrollment form (3 min).
- Staff submits the enrollment to PhilHealth for validation (2 min).
- Receive confirmation of enrollment. PhilHealth ID will be distributed upon availability (2 min).
