Special social services for mentally challenged individuals in Mapandan. Counselling, referral, and PhilHealth coordination.
Description
The MSWDO provides special social services for mentally challenged clients and other individuals requiring specialized assistance. Services include counselling, referral to other agencies and hospitals, preparation of Social Case Study Reports, and coordination for PhilHealth Point of Care benefits. The MSWDO assesses client eligibility and prepares referral letters to district hospitals and medical centers.
Requirements
- Certificate of Indigency from Barangay Captain
- Medical Certificate (if necessary)
- Birth Certificate
- Marriage Certificate (if married)
Procedure
- Register at the MSWD Office. Staff will refer the client for intake.
- Discuss the problem and provide relevant information. Staff interviews and assesses the client.
- Submit Certificate of Indigency, Medical Certificate, Birth Certificate, and whole body picture.
- MSWDO prepares referral letter or Social Case Study Report (SCSR).
- Receive the Social Case Study Report or Referral Letter.
- Proceed to other concerned agencies as necessary (District Hospital, PRU Hospital, Region 1 Medical Center).
- MSWDO coordinates for PhilHealth Point of Care eligibility assessment.
- Receive Certificate of Indigency for PhilHealth benefits.
