SB Financial Assistance

Description

The Office of the Sangguniang Bayan provides financial assistance to indigent residents of Mapandan. The client submits documents including a medical certificate, certificate of indigency, and valid ID. The SB staff receives and checks the documents, prepares them, and forwards to the Budget, Accounting, and Treasury Office for check preparation.

Requirements

• Medical Certificate (from Attending Doctor)
• Certificate of Indigency (from Barangay)
• Valid ID

Procedure

1. Submit your documents (Medical Certificate, Certificate of Indigency, Valid ID) to the SB Office.
2. SB staff receives and checks the submitted documents.
3. Staff prepares the documents and forwards them to the Budget, Accounting, and Treasury Office for check preparation.
4. Follow up with the Treasury Office for the release of the check.
