Sanitary Permit

Description

A sanitary permit is required for all businesses that handle food, beverages, or other consumable products to ensure compliance with health and sanitation standards. The permit is issued by the Municipal Health Office (MHO) after an inspection of the business premises to verify that health and safety regulations are met.

Requirements

  • Valid ID (government issued, employment, school, barangay certification)
  • Laboratory Tests: CBC, urinalysis, fecalysis, sputum exam, Hepatitis B (from RHU Primary Laboratory)
  • Chest X-Ray, PA (from government or private diagnostic facility)

Procedure

  1. Present official receipt from the Treasury Office.
  2. Laboratory examination of urine, feces, sputum is conducted by Medical Technologist (30 min).
  3. Refer to other laboratories for X-ray if needed (2 min).
  4. Refer result to MHO for interpretation (10 min).
  5. MHO does physical examination and interprets result (5 min).
  6. MHO prescribes medicines if needed (2 min).
  7. MHO signs the certification (2 min).
  8. List down certificate number on logbook and release certification (2 min).
