Apply for a new business permit at the BPLO in Mapandan. Includes endorsement to Engineering, MPDC, RHU, and BFP.
Description
A business permit is required for all individuals or entities intending to operate a commercial business within the Municipality of Mapandan. This permit is issued after compliance with documentary and safety requirements and upon payment of applicable fees.
Requirements
- Filled up UNIFIED Form / Community Tax Certificate (from BPLO/Treasury Office)
- DTI/SEC/CDA Registration (from DTI/SEC/CDA Office or Online Application)
- BIR COR, if available (from RDO Calasiao)
- Barangay Clearance (System Integrated)
Procedure
- Fill up the UNIFIED form and submit necessary requirements for verification (10 min).
- BPLO encodes data into the e-BPLS System.
- BPLO endorses applicant to various offices: Engineering (Occupancy Certificate), MPDC (Zoning Certificate), RHU (Health Certificate & Sanitary Permit), BFP (Fire Safety Inspection Certificate) (44 min).
- Pay fees and charges at the Treasury Office.
- Proceed to the Mayor's Office for issuance of Business Permit.
- Claim business plate and sticker at the Treasury Office (10 min).
