PWD Assistive Devices
Request assistive devices for PWDs from the MSWDO in Mapandan. Crutches, wheelchairs, and other devices available.
Description
The MSWDO provides assistive devices such as crutches, wheelchairs, prosthesis, and other gadgets to Persons with Disabilities (PWDs) to help them integrate into the mainstream of community life. The service covers orthopedically handicapped, visually impaired, and immobile individuals aged 0–59 years old. The MSWDO coordinates with PSWDO and DSWD Field Office I for funding and procurement of devices.
Requirements
- Barangay Certificate of Indigency
- Medical Certificate with doctor's recommendation for assistive device
- Whole body picture
Procedure
- Apply at the MSWDO. A family member may represent the client.
- Staff will interview and gather relevant information from the client or family member.
- Submit Barangay Certificate, Medical Certificate, and whole body picture.
- MSWDO conducts follow-up, home visit, and assesses eligibility and device needs.
- MSWDO prepares and submits documents to PSWDO/DSWD FO1 for funding and purchase.
- Receive the assistive device (crutches, wheelchair, prosthesis, etc.) upon availability.
