PWD Assistive Devices
Mag-apply para sa mga assistive device mula sa MSWDO para sa mga PWD sa Mapandan.
Paglalarawan
Ang MSWDO provides assistive devices such bilang crutches, wheelchairs, prosthesis, sa iba pang gadgets upang Mga Taong maaaring Kapansanan (PWDs) upang tulong them integrate into ang mainstream ng pamayanan life. Ang serbisyo covers orthopedically handicapped, visually impaired, sa immobile individuals aged 0–59 mga taon luma. Ang MSWDO coordinates kasama ang PSWDO sa DSWD Field Tanggapan I para sa pagpopondo sa procurement ng devices.
Mga Kinakailangan
- Barangay Sertipiko ng Indigency
- Medikal Sertipiko kasama ang doctor's recommendation para sa assistive device
- Whole body picture
Prosedura
- Mag-apply sa ang MSWDO. Isang family member maaaring represent ang client.
- Staff ay interview sa gather relevant impormasyon mula sa ang client o family member.
- Isumite Barangay Sertipiko, Medikal Sertipiko, sa whole body picture.
- MSWDO conducts follow-up, home bisitahin, sa assesses pagiging karapat-dapat sa device needs.
- MSWDO prepares sa submits mga dokumento upang PSWDO/DSWD FO1 para sa pagpopondo sa purchase.
- Tumanggap ang assistive device (crutches, wheelchair, prosthesis, etc.) upon availability.
