Description

The OSCA Death Aid Benefits provide financial assistance to families of deceased Senior Citizens. The benefit is processed through the MSWDO upon submission of the deceased's original death certificate, two photocopies of the OSCA ID, and surrender of the OSCA ID. The MSWDO prepares the application and submits reports to the Municipal Budget Office for fund allocation.

Procedure

  1. Register at the MSWDO. Staff will conduct intake interview and assess eligibility.
  2. Submit the deceased's original Death Certificate, two photocopies of OSCA ID, and surrender the OSCA ID.
  3. MSWDO prepares the Application Form and Certificate of Eligibility.
  4. MSWDO submits report with attached documents to the Municipal Budget Office for fund allocation.
  5. Follow up with the MSWDO for the status of financial assistance.
  6. Receive the OSCA financial assistance at the Municipal Treasury Office.
