Apply for a marriage license at the Mapandan MCR. Fee: ₱862.
Description
Registration and issuance of marriage license at the Municipal Civil Registrar. The MCR interviews the parties on their residence, verifies birth certificates, and processes the application. The client proceeds to Treasury for payment (₱862) and returns to MCR for document release. Additional requirements for delayed registration include negative PSA certification.
Requirements
- Municipal Form No. 90
- Community Tax Certificate (from Treasury Office)
- Parental Consent of father/guardian (from MCR)
- Certificate of Family Planning (from Mun Health Office)
- Certificate of Pre-Marriage Counselling (from MSWD Office)
- CENOMAR / Birth Certificate (from PSA)
- Legal Capacity (from DFA, if one party is a foreigner)
Procedure
- Apply for marriage license at MCR. Interview conducted on residence, birth certificates verified.
- Proceed to Treasury Office for payment of ₱862.
- Receive official receipt.
- Present OR to MCR Office. Staff advises on issuance of license.
- Secure copy of marriage license. Documents released.
