Register legal documents at the Mapandan MCR. Legitimation, acknowledgement, and RA 9255 services available.
Description
Registration of legal documents at the Municipal Civil Registrar. Covers Affidavit of Legitimation (for parents not married before child's birth), Affidavit of Acknowledgement (for fathers acknowledging children), and RA 9255 applications (use of father's surname). Fee: ₱315 for legitimation, ₱215 for acknowledgement and RA 9255. MCR examines documents, prepares attachments, and makes annotations on the registry book.
Requirements
- Affidavit of Legitimation (for legitimation)
- CENOMAR from PSA
- Certified copy of child's COLB from MCR
- Certified copy of parent's Marriage Certificate from MCR
- Affidavit of Acknowledgement (for acknowledgement)
- Affidavit to use the surname of the father (for RA 9255)
- Other supporting documents: baptismal certificate, insurance policies
Procedure
- For legitimation: Parents not married before child's birth submit requirements. MCR examines documents, prepares attachments, and annotates registry book (₱315, 20–25 min).
- For acknowledgement: Father acknowledges children. Same procedure as above (₱215, 20–25 min).
- For RA 9255: Parents apply to use father's surname. Same procedure (₱215, 20–25 min).
- MCR annotates and approves documents (5 min).
