Annotate civil registry documents based on court order at the Mapandan MCR. ₱515 for corrections and adoption.
Description
Annotation of Certificate of Live Birth (COLB) or Certificate of Death (COD) based on court order. Covers corrections of entries, annulment, presumptive death, and adoption. The MCR examines documents for authenticity and completeness, makes annotations on the certificate, and prepares other attachments. For adoption, an amended COLB is prepared. Fee: ₱515 for both services.
Requirements
- Certified copy of Court petition from PSA
- Court Order from PSA
- Finality and certification of registration where the court is functioning from PSA
Procedure
- Present court order for correction of entries / annulment / presumptive death. MCR examines documents (30 min, ₱515).
- MCR makes annotations on the certificate and prepares attachments (15 min).
- For adoption: Present court order. MCR examines documents and prepares amended COLB (20 min, ₱515).
