Annotation ng Court Order at Adoption
Mag-file ng court order annotation o adoption sa MCR sa Mapandan.
Paglalarawan
Annotation ng Sertipiko ng Live Birth (COLB) o Sertipiko ng Death (COD) batay sa court order. Covers corrections ng entries, annulment, presumptive death, sa adoption. Ang MCR examines mga dokumento para sa authenticity sa completeness, makes annotations sa ang sertipiko, sa prepares iba pang attachments. Para sa adoption, isang amended COLB ay prepared. Bayad: ₱515 para sa both mga serbisyo.
Mga Kinakailangan
- Certified copy ng Court petition mula sa PSA
- Court Order mula sa PSA
- Finality sa certification ng pagpaparehistro where ang court ay functioning mula sa PSA
Prosedura
- Present court order para sa correction ng entries / annulment / presumptive death. MCR examines mga dokumento (30 min, ₱515).
- MCR makes annotations sa ang sertipiko sa prepares attachments (15 min).
- Para sa adoption: Present court order. MCR examines mga dokumento sa prepares amended COLB (20 min, ₱515).
