Have your affidavit subscribed and sworn before the Mayor of Mapandan. ₱130 fee, 15 minutes processing.
Description
The Office of the Mayor administers the subscription of affidavits for residents of Mapandan. The client presents the draft affidavit, the Mayor or authorized representative administers the oath, and the document is signed and sealed. The client pays the required fee at the Treasury Office.
Requirements
- Draft Affidavit (printed in legal size bond paper)
- Valid Government-issued ID of the affiant
- Community Tax Certificate (Cedula) from Treasury Office
Procedure
- Present the draft affidavit and valid ID to the Mayor's Office.
- Staff reviews the document (5 min).
- Pay ₱130 at the Treasury Office and obtain official receipt.
- Present OR. The Mayor or authorized representative administers the oath and signs the affidavit (5 min).
- Receive the notarized affidavit (5 min).
