Declaring Unrevised / Missing Tax Declaration

Replace a lost or missing Tax Declaration at the Assessor's Office. ₱700 fee, 30 minutes processing.

Description

Property owners whose Tax Declaration has been lost, destroyed, or was never issued may apply for a replacement or new declaration with the Municipal Assessor's Office. The Assessor verifies the property records, conducts appraisal if needed, and issues a new Tax Declaration. This ensures the property remains in the tax rolls with accurate records.

Requirements

  • Affidavit of Loss (if Tax Declaration was lost or destroyed)
  • Valid Government-issued ID
  • Proof of ownership (Deed of Sale, Tax receipts, or other documents)
  • Blueprints or sketches of the property (if available)

Procedure

  1. Visit the Assessor's Office and submit all required documents.
  2. Staff receives and verifies the application (5 min).
  3. Assessor checks records and conducts appraisal if needed (15 min).
  4. Staff prepares the new Tax Declaration (5 min).
  5. Pay ₱700 at the Treasury Office and claim the Tax Declaration (5 min).
