Mag-rehistro ng pagkamatay sa MCR sa Mapandan.
Paglalarawan
Registration of death at the Municipal Civil Registrar. The solemnizing officer or authorized representative submits the Certificate of Marriage for verification. If the place of death is not in a hospital, the person who last attended the deceased reports. Late registration requires negative PSA certification. Fee for late registration: ₱215. MCR approves after 10 days of posting.
Mga Kinakailangan
- Accomplished Certificate of Marriage
- Negative certification from PSA (for late registration)
- Affidavit of delayed registration (for late registration)
- Certified copy of Marriage license (for late registration)
Prosedura
- Solemnizing Officer or authorized representative submits documents. MCR examines accuracy and completeness (10 min).
- In default of Solemnizing Officer, either or both contracting parties may report (10 min).
- For late registration: Verify from CRIS/Registry Book. If negative, submit requirements and prepare certificate (20 min, ₱215).
- MCR approves documents (after 10 days of posting).
- Secure copy of death certificate or late registration. Release documents (3 min).
