Paglalarawan
Registration of birth at the Municipal Civil Registrar. Covers three pathways: (1) Attending midwife/hilot reports child's birth — requires Marriage Certificate of parents and Mun. Form 102; (2) Unmarried parents — requires Community Tax Certificate; (3) Hospital administrator — requires Accomplished Mun. Form 102. Late registration requires negative PSA certification and supporting documents. Fees: Misc Fee ₱115 (standard), ₱215 (unmarried), ₱100 (hospital), ₱315–₱1,015 (late registration depending on age).
Mga Kinakailangan
- Marriage Certificate of parents from PSA (for attending midwife/hilot registration)
- Community Tax Certificate from Treasury Office (for unmarried parents)
- Accomplished Mun. Form 102 from MCR (for hospital registration)
- Negative certification from PSA (for late registration)
- Affidavit of delayed registration (for late registration)
- Supporting evidence: Baptismal Certificate, School records, ITR, Insurance Policy, Medical records, Barangay certification (for late registration)
Prosedura
- Attending midwife/hilot reports child's birth. MCR evaluates information and registers (10 min).
- For unmarried parents: Hospital admin or representative prepares documents (15 min).
- For late registration: Apply at MCR, verify from CRIS/Registry Book. If negative, submit requirements. Data typed in Certificate of Live Birth (15 min typing).
- MCR approves registration (after 10 days of posting for late registration).
- Secure copy of birth certificate. MCR releases documents (3 min).
