Mag-claim ng financial assistance payment sa Mapandan.
Paglalarawan
Payment of financial assistance and other claims for indigent residents of Mapandan. The Treasury Office verifies documents, checks signatures, and releases payment. Cash is released for amounts ₱3,000 and below; checks are issued for amounts exceeding ₱3,000. An authorization letter is required if a representative claims on behalf of the beneficiary.
Mga Kinakailangan
- Disbursement Voucher with complete supporting papers
- Official Receipt from suppliers/contractors
- Authorization letter (if claiming through representative)
- Valid ID
Prosedura
- Go to Counters 3 and 4. Mention your claim and provide ID and/or authorization to claim.
- Staff checks if documents are available for release (10 min).
- Sign on the received page. Staff verifies signature and identification (10 min).
- Receive cash if ₱3,000 and below (recorded in Petty Cash Fund).
- Receive check if more than ₱3,000 (recorded in Cheque Disbursement Book).
